The Case Manager for Independent Living provides case management for homebound seniors, 60 years of age and older, through assessment of physical, psychological, social, and financial needs. A care plan, and coordination and delivery of service is then developed to best fit the senior’s needs. The Case Manager consults with public officials, agency representatives, and other professions to arrange for care and services. Cases are followed long-term by performing follow-up assessments and ensuring clients receive needed services.
Home visits are made a minimum of four times per year to each client, with many clients receiving additional visits in order to receive assistance paying bills, navigating medical care, and completion of paperwork for various programs.
